General Record Checklist Applicable to NCLB and Title I
- The electronic NCLB Monitoring Instrument via the ISBE IWAS system must be completed prior to your scheduled on-site visit. (instructions)
- The electronic NCLB Title I Comparability analysis should be completed prior to your scheduled on-site visit. (instructions)
- LEA’s board policies for Equivalency of Services among Schools, Parental Involvement, Unsafe School Choice, Public School Choice and Military Access to School Records.
- Written Multidistrict Agreement (if applicable).
- LEA’s compliance with project application and implementation e.g. approved copy of current application and amendments, teacher or administrator reviews or notes.
- Building’s Title I Plan.
- Building’s School wide Plan (if applicable).
- Building’s Target Assistance Plan (if applicable).
- LEA’s plan for implementing the Parents Right-to Know provision e.g. letter to parents, board policy.
- Guidelines related to equipment purchases and inventory.
- Evidences that the federally funded services supplement, rather than supplant services and programs provided with state and local funds.
- Documentations to support compliance with consultation and participation requirements.
- Documentations to support compliance with the requirements of private school participation.
- Documentations that the LEA’s is in compliance with the ‘Stevens Amendments (if applicable).
- Documentations to support the LEA’s compliance to maintain a homeless education liaison.
- Records to substantiate funded staff are providing education services to eligible pupils.
- Documentations to support LEA’s compliance with maintenance of effort.
- LEA’s policy for comparability of services.
- Documentations that supports the LEA’s plan to coordinate services with the Early Childhood Program.
- Documentations that supports the LEA’s compliance to provide Supplemental Educational Services.
- School Choice-board policy, notification letter, list of pupils.
- Non-public allocation breakdown by school
- Free and reduced lunch data
- NCLB Highly Qualified Report via IWAS
- SES letters, provider contracts, student tutorial plans and other SES related documentation to support compliance with SES provisions
- Annual parent meeting documentation to explain the Title I Program (i.e. meeting agenda, minutes, sign-in sheets)
