Application Process

In order to qualify for transcript evaluation, an applicant must meet the following criteria:

1. If applying for a teaching certificate please complete an "Application for Teaching Certificate" (form 73.03C), appropriate for the type of certificate being sought:

2. If applying for an Administrative or School Service Personnel certificate please complete an "Application for School Service Personnel or Administrative Certificate" (form 73.03D), appropriate for the type of certificate being sought

3. Submit a non-refundable fee for each certificate requested to your Regional Office of Education.  Please contact your regional office to determine who the fee should be made payable to, and the appropriate amount.

4. Submit official transcripts from all colleges attended.   Transcripts must bear the seal of the institution and the signature of the registrar.

5. Submit a signed and sealed State Approved Program Verification Form (73-05) completed by the institution where the approved program was completed.

6. Submit a copy of both sides of a comparable, valid, out of state certificate.

7. Submit any original letters of teaching experience from your employer (s).  Letters need to state the grade level/subject and number of months of full-time teaching experience and must be signed by a district administrator or superintendent.

8. If you hold a valid, comparable, out of state certificate and wish to receive an Illinois Provisional Certificate, then submit a completed "Request for Issuance of a Provisional Certificate" (form 73-63) 

9. If you hold a valid, comparable, out of state certificate and wish to be considered for a testing waiver, then submit Confirmation of State Certification Tests Taken (73-99) completed by the state agency, college or university.

Upon receipt of your certificate, you will be notified of the amount of the registration fees that you must submit to your regional superintendent's office.
