Household Eligibility Applications – The following forms are user-friendly. You can input school/district information on the form by simply clicking on the various fields. The forms can then be printed for your use, but cannot be saved unless you have an advanced version of Acrobat Reader. NOTE: If you operate the National School Lunch and/or School Breakfast Programs, you are automatically enrolled in the Illinois Free program as well. In this case, use the National School Lunch and School Breakfast Programs Application for all students. If you do not operate a national program and are only in the Illinois Free Program, use the applicable Illinois Free Application.
