Thank you for your interest in becoming a sponsor of a school-based
child nutrition program! School-based child nutrition programs
include the following: National
School Lunch, School
Breakfast,
Afterschool Care Snacks, Special
Milk, and Illinois Free
Lunch and Breakfast.
Prior to completing the Program Participation Request Form
for School-Based Child Nutrition Programs, prospective sponsors
are required to become familiar with all of the program regulations
and policies regarding program administration and operation.
Be sure to thoroughly review the following documents, menu

After reviewing all program regulations, policies, and other
relevant information, complete the program participation request
form and mail it along with all required documentation as
noted on the form. It is recommended that you retain a copy
of the form for your files. If you have questions or need

Once approved as a sponsor in the National School Lunch Program
(NSLP) and/or School Breakfast Program (SBP), it is highly
recommended that you attend one of our workshops designed
for new sponsors: NSLP Basic Training—New Sponsor/New
Staff Orientation. For information on workshops regarding
NSLP and/or SBP program administration, click on the link
below. The workshop/training schedule link provided on that
page will allow you to select a date and location that works
best for you. Once you are on the workshop schedule page,
be sure to scroll down to the correct title (do not select
the CACFP, Child and Adult Care Food Program, trainings).
Once approved as a sponsor in the Illinois Free and/or Special
Milk Program only, please review the materials available
online or contact our office at 800-545-7892 for technical
assistance on program administration responsibilities.
