0-3 Prevention Initiative Program and Preschool for All Program



Required Data Reporting

Prevention Initiative (PI) grantees must report data for individual students, i.e., 0-3 infants and toddlers, in the Student Information System (SIS). Go to http://www.isbe.net/sis/html/data_elements.htm to review required data elements, including the “Birth to 3” and “Caregiver Demographic” data elements (under “Early Learning”). These data must be submitted according to key dates posted at http://www.isbe.net/sis/html/key_dates.htm.

Starting with FY 2013, the “0-3 Prevention Initiative Program Record” paper data collection (ISBE form 41-85) has been discontinued. PI program data related to the extent to which parent/family objectives outlined in the Individual Family Service Plan (IFSP) are achieved will be reported via a new IWAS system, the “0-3 Prevention Initiative – Outcomes Questionnaire” system. In addition, the “Prevention Initiative – Parent Evaluation” IWAS system has been redesigned and renamed the “0-3 Prevention Initiative – Parent Questionnaire” system. Both the “0-3 Prevention Initiative – Outcomes Questionnaire” system and the redesigned “0-3 Prevention Initiative – Parent Questionnaire” system will be available for data entry on January 7, 2013. For both systems, the deadline to submit FY 2013 data is July 1, 2013. Please refer to the questionnaires below for the data to be reported for FY 2013.

These data must be submitted according to key dates posted at http://www.isbe.net/sis/html/key_dates.htm.

Starting with FY 2013, the “Preschool for All Program Record” system in IWAS has been discontinued. “Pre-K Follow-Up” reporting in SIS has also been discontinued
